Forms:
Forms can be used to take input and deliver it to a database, an email, or a Web page. Forms are particularly useful for collecting customer information. In such a case, specific information such as name, email, or billing address may be required before a form is processed.

Search engines use a form to collect search criteria and return matching results. Bulletin and message boards display input for others to see and allow searches of past messages.

Examples:
Contact Form- This form uses an auto responder to confirm the message and sends the response to this site's Web master.

Dialectizer- This humorous Web site uses a form to receive input text or a URL and then translates the text of either to an English dialect such as redneck, jive, cockney, and more.
