Opening the album cover, two more clues are revealed. The first comes as “the patient”. He sits still in a wheelchair towards the right, and he is the protagonist in The Black Parade story. From observation, one can see he is terribly ill. His complexion his a ghastly white; he slouches in his seat, showing weakness, and his cheekbones protrude from his face. The clean-shaven head, pallid skin, and evident weakness exhibit cancer symptoms. Cancer plagues the body, causing it to slowly deteriorate. During this time, the patient has the opportunity to choose whether to embrace the inevitable or go down swinging. Physically speaking, he represents death in an obvious way. Any viewer could see this man isn’t doing so well and will probably die. Despite his cancerous state, the audience isn’t meant to dwell on their sorrows over this man. Instead, the band encourages the audience to look at them, since they positioned themselves at the center, leaving “the patient” to sit aside.
