If you would like me to create custom labels for your lab, please email me the details, such as label type, label sheet type, etc., and I will create just about anything. I also have the high resolution images as separate image files if those are so desired for use in other size or brand of label.
Also, I have a thing about forms and their ease of use compared to typewriters and handwritten papers...I just can't stand them! I like to create custom form fields in either Microsoft Word or Adobe Acrobat (PREFERRED). The difference for the normal user is the ability to SAVE documents with custom form field entries. If you do not have Adobe Acrobat Professional, you cannot save a form with the form fields "filled-out." If you do, great! If you don't, not to worry...I can create custom Microsoft Word forms that can be saved with fields filled out.

Original Copies of these forms (without updated form fields) can be found on the FedEx website. These forms have been customized to make them easier to fill out and therefor more than likely filled out correctly. All form fields are available for use, but please be careful...the "Shipper's Declaration of Dangerous Goods" form has been partially filled out with information specific to the University of Florida. Feel free to use these forms for your shipments, change them to suit your needs and share them with others.
If you don't want to use purchased blank labels...you don't have to. Simply print these labels on regular paper and cut out the individual labels. You will have to tape the label to the package and completely cover the label with clear packaging tape so that the ink on the label does not wear/wash off! Please to this for all labels to ensure a successful delivery!

Shipper's Declaration of Dangerous Goods
Are you shipping an Infectious Substance - Category A ???
Are you shipping a Hazardous Chemical (e.g. fixative or preservative or a genetically modified microorganism) at a quantity greater than that allowed with the use of the "Dangerous Goods in Excepted Quantities Form???"
If so, you need this form filled out and printed in:
(1) TRIPLICATE, and
(2) COLOR! (Red lines need to be RED!)
If you are keeping "paper" copies of your forms (and you probably should since an FAA inspector will want to see them - trust me, I know), you'll want to print out at least one more copy for your records...unless you can guarantee your computer won't crash and erase all of the documents on it...can you guarantere that???

Dangerous Goods in Excepted Quantities
Are you shipping a Hazardous Chemical at a total volume less than or equal to the maximum quantity listed on this table?
If so, you need this form filled out and printed in:
(1) COLOR! (Red lines need to be RED!)
(2) This cannot be less than 100mm x 100mm (4" x 4")!
Again, a color copy is needed for the package and why not run off another copy for your records, huh? What else are you going to do?

Blank Shipper and Reciever Labels
Do you have sheets of 2" x 4" labels just sitting around waiting to be fed through that $900.00 printer of yours? Well, I do...and I use these labels to make my boxes look even more professional (as if that were possible). I'm working on an additional Word Document that can be filled out before printing. BUT, in the mean time, you can print these out and fill them in after putting them on the package.

UN 3373 - BIOLOGICAL SUBSTANCE CATEGORY B
Are you shipping an Infectious Substance not listed on the list of Category A Infectious Substances?
This document is formatted for 5"x3" labels (AVERY 5168)
Also required by the packaging instruction (650) are Box Arrows on two (2x) sides of the box and:
Are you including DRY ICE in your shipment??? If so, you'll also need a label for that...as dry ice is a dangerous good with its own UN Number and harzardous classification...see below.
NOTE: The proper labeling for a Dry Ice shipment must include the Dry Ice UN Number (UN 1845), the amount of Dry Ice included in the package, AND the Class 9 hazardous label (diamond with black and white stripes). See below about these.

UN 1845 - DRY ICE - Full Sheet
"My latest and greatest"
This label includes everything you need for shipping dry ice in a package. You must have the:
(1) UN Number (1845)
(2) 4" x 4" Diamond with black and white stripes AND harzard class number for dry ice: "9".
(3) Net Weight (Kg) of Dry Ice included in package
...and this label has it all!

Double Arrow Up Marking Labels
Use these on opposite sides of your package to identify its upright orientation. If you have purchased specialized packaging (insulated, etc.) you may already have these on the box.
These are setup on a Microsoft Word document using the AVERY 5168 5"x3" label sheet and can be printed on regular paper as well.
